Corporate Wellness Survey

Workplace wellness programs are becoming increasingly popular worldwide as employers seek to forge partnerships with the health team encouraging the development of healthy working environments. The main goals of this form of health promotion are to improve employee health by reducing the risk of chronic lifestyle problems such as heart disease, diabetes, hypertension, obesity, cancer and HIV/AIDS; to improve productivity and performance and to reduce health care costs.

As a Corporate Wellness Consultant working in the region, I am seeking to empower suitable designated persons from within organizations who could spear head or assist in developing a wellness program specific to the organization’s vision and culture.

In the first phase of the implementation process for corporate wellness, we must gather information about the current state of your working environments. The attached questionnaire has been designed to include the capture of information, needed to make a determination of the costs, which could have been prevented were a full-scale wellness programme to be put in place and functioning for over a 3 – 5 year period, within your organization. The survey should also give us a reference point for the purposes of planning a wellness development-training seminar suitable to the population.

We welcome your assistance by completing the attached questionnaire, in particular Section B namely, the number of employees and the average annual health claim per employee and return by fax or email to me at dennylu57@yahoo.com by March 31, 2006. In return, when I have done the evaluation, I will send you a brief report and an estimate of the preventable health costs for your organization using the Preventable Claims calculator developed by Yen & Edington, from the Steelcase Study (1991).

It is my desire to facilitate a successful Wellness Programme development training seminar in May 2006 in Trinidad as was done with 59 persons from 35 organizations in April last year in Jamaica.
