Personal Wellness Profile™ (PWP)
The award-winning Personal Wellness Profile TM (PWP) is our most popular and comprehensive health assessment tool. It is a general health assessment and profiling application for proactive population management, and is available in both online and desktop versions. The PWP system provides you with the power to assess health risk, target specific groups, track progress, and evaluate cost-benefit. PWP Operation
The PWP health risk assessment system is available in the Comprehensive Plus Edition, Concise Plus Edition, and special populations that include, Senior, Teen, Spanish, Spiritual, and French-Canadian.
PWP Comprehensive Plus provides a thorough wellness-based health assessment addressing all major lifestyle factors, personal and family history, symptoms, functional health status and quality of life, health interests, readiness to change, and self-efficacy. Participants complete a 75-question questionnaire in 25 to 45 minutes, not including 30 optional clinical and fitness test data such as blood pressure, lipid values, glucose, PSA, body fat, and others that determines current health status. Extensive personal, aggregate, and administrative summary reports can be produced with virtually unlimited selection criteria for targeting and subgroup outcomes analysis. Assessment data is acquired through paper and pencil questionnaires that are scored by an OpScan® scanner and then placed into the PWP system for data mining and report generation.
PWP Concise Plus is a condensed version of the Comprehensive Plus, and is used in settings where a shorter, simpler questionnaire is important. The Concise Plus is used most frequently by managed care organizations with new enrollees and returning members in mail-based contact to collect health risk information for targeted follow-up programs. The data produced by the Concise Plus also assists in the development of lifestyle improvement programs, health enhancement, and prevention services to address NCQA quality criteria.
Special Populations
Wellsource understands that a general population assessment is sufficient in many cases, although recent health findings have motivated us to develop more specialized assessment systems including the Teen, Senior, Spanish, Spiritual, and French-Canadian Editions. Each of these special population editions maximizes program efforts by scientifically identifying those participants that are most likely to take an interest in improving their health.
